Application Eligibility

Funding eligibility
Ministry of Health Those who are currently enrolled as a PGY2 in a Canadian 
Family Medicine Residency Program at the time of application.

Residents must start their PGY3 year within 12 months of
completing their PGY2 year.
Ministry of Health Re-Entry Program

Practicing physicians must apply separately through the Ministry of Health and Long Term Health Re-Entry Program. 

Selection is determined by application program review and program capacity.

How to Apply


  1. Completed Online Application Form
  2. Letter of Intent (to the attention of the Program Director of the program of choice) addressing objectives and motivations for pursuing additional training
  3. Current CV
  4. Copy of Medical Degree 
  5. Three (3) references*, one which must be from your Residency Program Director or Site Director.
  • Please note that referees will be submitting references online via the online application system. These requests are generated electronically once an application is submitted and a confirmation is sent to the applicant. Referees will have two (2) week from the date of receiving the notification request to submit a letter of reference (e.g. for applications submitted and approved on October 4, 2019, referees will have until October 18, 2019 at the latest to submit their letters of reference). Program Directors will NOT review applications without reference letters.
